Use this comprehensive form to obtain a clear picture of the Claimant’s employment history and expectations.

Once you have the full occupational history you may find it helpful to prepare a brief assessment  setting out pre and post accident earnings.

Assessment:

Use this form to briefly set out the Claimant's expected career progress and earnings, and what the residual earnings are likely to be. Selected values obtained from Askpay should be included in the appropriate area.
